Position: Temporary Care Coordinator Specialist
Reports To: Program Manager
Department: Integrated Care
Key Responsibilities and Duties:
The role involves delivering critical care coordination to individuals directed to specific specialized programs. Key duties include evaluating client needs, identifying suitable human service initiatives and community assets through CareSuite, and facilitating referrals to service providers. The specialist is also tasked with providing program-specific follow-up, engaging with partner organizations to clarify 211 services, and monitoring diverse project results to ensure client needs are met.
Deliver comprehensive assessments and safety planning for individuals at-risk of or impacted by local disasters.
Provide referrals and advocacy for clients as needed.
Participate in case conferencing with partner agencies to coordinate care, prevent duplication of services, and support smooth transitions between providers.
Follow up with clients and referral organizations to determine if each client has accessed services.
Maintain record of all interactions with clients and their care team (other social service providers, family supports, and medical providers, etc.) in CareSuite.
Serve as a liaison between 211 LA County and other partnering agencies as needed.
Travel as needed to achieve Agency objectives; work evenings, weekends, and/or holidays may be required.
Be available for an undetermined schedule to respond to county emergency response in the event of an unforeseen emergency or disaster.
Perform special assignments and other duties as assigned and according to established Agency mission and values.
: Qualifications:
Previous experience with Care Coordination or Case Management.
Collaborate professionally with colleagues.
Engage and communicate effectively with individuals from diverse cultural, age, and socioeconomic backgrounds.
Demonstrate strong organizational capabilities and the ability to manage multiple concurrent tasks with shifting priorities.
Possess advanced oral, listening, written, and presentation skills, including proficiency in grammar, style, and content development.
Document and maintain professional, comprehensive, and concise case management records.
Operate independently while exercising initiative and sound professional judgment.
Contribute effectively to a collaborative, team-based work environment.
Acquire proficiency in and utilize various computer software and hardware systems.
Possess and maintain a valid driver's license and the required proof of insurance.
Remain available for a flexible schedule to support county emergency response efforts during unforeseen disasters or emergencies.
Demonstrate professional bilingual proficiency in Spanish.
Compensation: $25.50 an hour
