The Role:
As a Nurse Practitioner (Telehealth) at Greater Good Health, you'll be at the heart of our mission to provide exceptional care to patients. Your responsibilities will include:
- Coordinating continuity of care, prevention and avoidance of complications, and close clinical follow-up for patients recently discharged from the ER or hospital, under the direction of the patient's primary healthcare provider(s)
- Counseling patients and families with education around their discharge plan, medications, and warning signs to watch for
- Conducting annual wellness visits through telehealth
- Thoroughly documenting medical histories, symptoms, and medication information
- Administering various tests and screenings
- Collaborating with the Primary Care team to ensure holistic patient care
- Ensuring timely completion of patient records, with access to electronic medical records and portals
- Maintaining strict confidentiality in line with HIPAA regulations
What You Can Expect:
This is a full-time, contract-based Nurse Practitioner position (5 days/week, 8am–5pm). The initial term is until 12/31/2026, with the possibility of conversion to a full-time employee role.

Experience and Qualifications
- Current California Nurse Practitioner license
- Nurse Practitioner certification from either the ANCC / AANP
- 2 + years of clinical experience as a NP in either Primary Care, Family Medicine, Internal Medicine, Gerontology or Palliative Care. Home care experience is a plus
- Working knowledge of chronic care management and various medication options
- Current DEA licensure / Prescriptive Authority post-hire
- Active CPR/BLS Certification
- Current California Driver’s License
