Required: Active Washington Nurse Practitioner license + registered/enrolled with Washington Medicaid.

What You’ll Do

• Conduct Comprehensive Health Assessments through telehealth
• Review medical history, medications, and preventive care needs
• Document HCC (risk adjustment) findings accurately
• Close HEDIS (quality measure) care gaps
• Use ICD-10 and CPT II codes for documentation
• Deliver clear care plans and follow-up guidance
• Complete documentation on time

Must-Haves

• Active Nurse Practitioner license in Washington
• Registered or enrolled with Washington Medicaid
• 1+ year Family Medicine or Internal Medicine experience
• Experience caring for Medicare and Medicaid populations
• Familiarity with HEDIS and risk adjustment workflows
• Available at least 24 hours per week

Nice to Have

• Additional active state NP licenses
• Value-based care experience
• Annual Wellness Visit experience
• HCC risk adjustment documentation experience
