Working Hours: Part Time & Full time (37.5 hours) contracts available with a minimum requirement of 21 hours over 3 days

Purpose of Role:

To provide a service to ILS Clients that is devoted to the coordination, rehabilitation, care and support of people with complex clinical needs. To provide a service that aims to facilitate Client’s independence and improve their quality of life; whilst acknowledging safety issues.

Key Responsibilities:

Case Manager duties will include:

  • Assessments of clients’ function & needs

  • Producing accurate and detailed formal reports including identification of time required to undertake tasks

  • Planning and reviewing goals

  • Providing risk assessments and identifying safeguarding concerns

  • Acting as an advocate for clients

  • Supporting clients and their families

  • Liaising with professionals and other agencies on the client’s behalf

  • Recruiting and managing Care Teams

  • Commissioning & coordinating multi-disciplinary teams

  • Completing and producing all necessary paperwork in relation to the Client as required by the Care Quality Commission

  • Attendance at court as required

Additional Responsibilities:

  • To follow the framework for implementing Case Management when a client is allocated to them

  • To receive clinical mentorship

  • To meet business objectives

  • To engage in the ILS Learning & Development Programme and maintain your CPD

  • To complete tasks within accepted time frames

  • To attend ILS Company/Connect events

  • To complete all necessary paperwork / tasks in accordance with ILS policy and procedures

  • To maintain the reputation of ILS

  • To support with the induction of new Case Managers by enabling them to accompany you on client visits/attend online client meetings where appropriate

  • To recognise and report safeguarding concerns promptly in line with ILS Case Management policies and statutory guidance, maintain accurate records, work collaboratively with safeguarding partners, and complete mandatory safeguarding training

Person Specification:

Essential Criteria

  • Qualified healthcare or social care professional (e.g. Occupational Therapist, Physiotherapist, Nurse or Social Worker) with current HCPC, NMC or SWE registration and an up-to-date CPD portfolio

  • Clear understanding and application of the principles of the mental capacity act 2005

  • Experience undertaking complex assessments, risk assessments, and supporting clients with complex needs

  • Strong understanding of the UK health and social care system and rehabilitation principles

  • Proven ability to work autonomously, manage competing priorities, and meet deadlines

  • Experience working within community-based settings, maintaining professional boundaries and achieving positive client outcomes

  • Excellent written and verbal communication skills, including report writing and case documentation

  • Competent in Microsoft Office and confident using digital systems

  • Ability to lead, motivate and coordinate multidisciplinary teams

  • Flexible approach to travel and working arrangements, with access to own transport

Desirable Criteria

  • Previous case management experience

  • Experience working with clients with ABI, spinal injury, cerebral palsy, stroke, or mental health needs

  • Knowledge of care package development, staff recruitment, supervision, and performance management

  • Experience liaising with solicitors, deputies, and the Court of Protection, including attendance at court proceedings

  • Awareness of relevant Health & Safety and Employment legislation

  • Experience working remotely and creatively developing solutions that go beyond standard service provision
