Social Worker II


The Social Worker II (MSW) provides advanced psychosocial support and care coordination for individuals living with complex chronic conditions within a mission-driven nonprofit environment. This role supports patients and families in navigating the medical, emotional, and socioeconomic challenges associated with complex chronic conditions.

Working collaboratively with interdisciplinary teams and community partners, the Social Worker II assesses complex needs, develops person-centered care plans, and connects individuals to critical resources that improve health outcomes and quality of life. This position also supports the organization’s mission through advocacy, education, and outreach across diverse and underserved populations.

The Social Worker II position is a full-time remote position. The successful candidate would have excellent written and verbal communication, organizational skills, and attention to detail. A minimum of two (2) or more years of experience in social work, preferably in healthcare, chronic disease management, or nonprofit settings. Preference will be given to applicants with experience supporting dialysis patients and/or individuals with end-stage renal disease (ESRD). A Master's degree in Social Work with a current licensure as a Clinical Social Worker or eligibility for licensure. This position requires travel up to 20%. The target compensation for this position is $55,000 - $73,000.
