The Social Worker — provides intensive, housing focused case management to Veterans experiencing homelessness or at risk for homlessness. Each Social Worker will maintain a caseload of no more than four active Veterans, allowing for frequent contact, individualized support, persistent outreach, and hands-on assistance through every stage of the housing process. The Social Worker conducts intake and comprehensive assessments; develops and regularly updates individualized service plans (ISPs); provides housing navigation; coordinates medical, behavioral health, benefits, legal, employment, transportation, and other supportive services; monitors progress; and facilitates placement into safe, appropriate, and sustainable permanent housing. The position serves as a consistent point of contact and advocate for each Veteran, using trauma-informed, person-centered, culturally responsive, harm-reduction, and housing-focused practices
