We are seeking a Licensed Practical Nurse (LPN) to join our remote Chronic Care Management team in an outbound, call-center-style environment. The Outbound LPN will proactively contact patients to support ongoing care management, assess their needs, follow established care plans, and help ensure continuity of care.
This position is ideal for an LPN who is comfortable communicating with patients by phone, managing multiple interactions throughout the day, documenting accurately, and working in a technology-driven remote environment.
• Make outbound calls to patients as part of ongoing Chronic Care Management services.
• Conduct monthly care management outreach and follow up with patients according to established care plans.
• Support continuity of care by identifying patient needs, barriers, and changes in condition that may require additional attention or escalation.
• Communicate with patients professionally and compassionately while maintaining a call-center-style workflow.
• Follow established care plans and care management protocols.
• Document patient interactions, updates, and follow-up activities accurately and timely.
• Maintain consistent outbound call activity and meet established productivity and quality expectations.
• Escalate clinical concerns or issues according to established protocols.
• Protect patient confidentiality and comply with HIPAA requirements.
• Participate in virtual meetings and team communications using Google Workspace, Gmail, and Google Meet.
• Meet established Key Performance Indicators (KPIs) for quality, productivity, attendance, and other performance measures.
• Active LPN license with a Compact State License.
• Must be licensed to practice in applicable states within the organization's service footprint.
• New York and California licensure are required/accepted as applicable to the assigned patient population.
• Previous nursing, healthcare, care management, patient outreach, or call-center experience preferred.
• Strong verbal and written communication skills.
• Comfortable making a high volume of outbound calls.
• Strong organizational skills and attention to detail.
• Ability to follow care plans, workflows, and established protocols.
• Basic computer proficiency and ability to navigate multiple applications.
• Must successfully complete a technology assessment, including typing and basic computer skills.
• Must have a dedicated, private workspace suitable for handling protected health information.
• Workspace must have a door and allow the employee to maintain HIPAA compliance.
• Must have a reliable high-speed internet connection.
• Must be able to connect directly to the home modem/router using a hard-wired Ethernet connection.
• A 30-foot Ethernet cable will be provided to facilitate the hard-wired connection.
• Company-provided equipment includes a desktop computer and two monitors.
• Employees may be required to share their screen during work activities or support sessions.
Employees will use Google-based tools and other company systems, including:
• Gmail
• Google Meet
• Google Workspace (G Suite)
• Company-provided desktop and dual-monitor setup
• $18.25/hour base pay
• +$1.00/hour bilingual differential, where applicable
• Up to $400/month KPI-based incentive

This is a remote, call-center-style nursing position. The successful candidate should be comfortable spending a significant portion of the workday communicating with patients by phone, working toward productivity and quality goals, documenting interactions, and maintaining a consistent workflow.
