About the Role:

The Remote Care Nurse II plays a critical role in delivering high-quality nursing care to patients from a remote setting, ensuring their health and wellness are effectively managed through virtual interactions. This position involves comprehensive patient assessments, care planning, and coordination with multidisciplinary teams to optimize patient outcomes while adhering to established clinical guidelines. The nurse will utilize telehealth technologies to monitor patient progress, provide education, and support self-management of chronic conditions, thereby reducing hospital readmissions and improving quality of life. This role requires strong clinical judgment and the ability to respond promptly to patient needs, emergencies, or changes in condition remotely. Ultimately, the Remote Care Nurse II contributes to a patient-centered care model that emphasizes accessibility, continuity, and personalized support across diverse populations within the United States.

Minimum Qualifications:

  • Current and unrestricted Registered Nurse (RN) license in the United States.
  • Minimum of two years of clinical nursing experience, preferably in case management, chronic disease management, or telehealth.
  • Proficiency with electronic health record (EHR) systems and telehealth technology platforms.
  • Strong communication skills to effectively engage with patients and interdisciplinary teams remotely.
  • Ability to work independently in a remote environment while managing multiple patient cases efficiently.
