Join the team as a Remote Care Manager focusing on the Engagement and Recovery Initiative. In this role, you serve as the essential link between various treatment components, ensuring that adult and child clients are supported from their initial intake through their long-term recovery journey. This position utilizes a strength-based model to provide comprehensive care coordination and transition support.
Key Responsibilities
Client Engagement: Conduct timely post-intake follow-up calls to ensure a positive experience and explain the value of care management services.
Assessments & Planning: Facilitate initial screenings, assist in developing person-centered care plans, and secure necessary consents for service coordination.
Care Coordination: Act as a liaison between physical and behavioral health providers, helping clients schedule appointments and overcoming barriers to treatment compliance.
Ongoing Support: Maintain regular weekly contact with clients to reinforce engagement, monitor progress, and ensure services meet the client's identified needs.
Transitional Care: Facilitate the successful transition of clients between initiative teams and local community centers, ensuring continuity of care.
Holistic Advocacy: Connect clients with essential community resources such as housing, food, and employment, while providing education on independent living skills.
