In this role, you serve as the essential link between various treatment components, ensuring that adult and child clients are supported from their initial intake through their long-term recovery journey. This position utilizes a strength-based model to provide comprehensive care coordination and transition support. Key Responsibilities Client Engagement: Conduct timely post-intake follow-up calls to ensure a positive experience and explain the value of care management services. Assessments & Planning: Facilitate initial screenings, assist in developing person-centered care plans, and secure necessary consents for service coordination. Care Coordination: Act as a liaison between physical and behavioral health providers, helping clients schedule appointments and overcoming barriers to treatment compliance. Ongoing Support: Maintain regular weekly contact with clients to reinforce engagement, monitor progress, and ensure services meet the client's identified needs. Transitional Care: Facilitate the successful transition of clients between initiative teams and local community centers, ensuring continuity of care. Holistic Advocacy: Connect clients with essential community resources such as housing, food, and employment, while providing education on independent living skills.
