The Re-Entry Care Coordinator supports individuals transitioning between care settings who are justice-involved and preparing for release from Department of Corrections or county detention facilities by providing care coordination and transition planning services. The role develops transition plans, aligns services across providers and community partners, and addresses barriers to support successful and stable transitions.

Required:
Fully licensed RN who is licensed to practice in the state of NC by the NC Board of Nursing who also has four (4) years of full-time experience with the MH/DD/SA population served. OR
