The physician will provide longitudinal primary care to a panel of medically complex older adults via a virtual-first model. Additionally, they will supervise and support nurse practitioners and physician assistants while collaborating with an interdisciplinary team.
Seniority Healthcare is seeking a part-time Primary Care and Supervisory Physician to provide direct patient care and clinical supervision across the organization’s primary care and COMPASS service lines.
This is a clinical position with two primary responsibilities:
Providing longitudinal primary care to a defined panel of medically complex older adults
Supervising and supporting nurse practitioners and physician assistants throughout Seniority Healthcare
The physician will care for patients in senior living communities and home-based settings through a virtual-first model. Visits are primarily conducted through audiovisual telemedicine, with audio-only visits used when clinically appropriate and permitted.
In addition to managing an individual patient panel, the physician will complete required supervisory visits, provide clinical guidance to advanced practice providers, and support complex medical decision-making across the practice.
This position is well suited for a physician who values long-term patient relationships, enjoys caring for geriatric and medically complex populations, and is comfortable serving as both a primary care provider and physician leader within a collaborative care model.
Seniority Healthcare provides comprehensive medical care to older adults in senior living communities and home-based settings. Its services include primary care, behavioral health, palliative care, and the COMPASS Program.
COMPASS—Collaborative Outpatient Management for Palliative and Aging Support Services—provides coordinated, longitudinal support for medically complex older adults. The program is designed to reduce care fragmentation, prevent avoidable hospital utilization, and improve patient experience and quality of life.
Maintain a primary care panel of approximately 150–200 patients
Provide comprehensive, longitudinal medical care to medically complex older adults
Conduct new-patient, follow-up, chronic care, transitional care, and acute visits
Deliver care through audiovisual telemedicine and audio-only visits when clinically appropriate
Evaluate changes in condition and develop appropriate treatment plans
Manage chronic illnesses, medications, preventive care needs, and care transitions
Coordinate care with specialists, hospitals, senior living teams, home health agencies, hospices, and other community-based providers
Participate in advance care planning and goals-of-care discussions
Complete clinical documentation accurately and within established timeframes
Help reduce avoidable emergency department visits and hospitalizations through proactive medical management
Provide clinical supervision and support to nurse practitioners and physician assistants across Seniority Healthcare
Complete physician supervisory and collaborative visits as required
Review complex cases and assist with diagnostic and treatment decisions
Serve as an accessible physician resource for advanced practice providers
Support escalation pathways for changes in condition, urgent concerns, and higher-acuity patients
Promote consistent clinical standards, documentation quality, and evidence-based practice
Participate in case reviews, clinical education, and performance-improvement initiatives
Support advanced practice providers while encouraging appropriate clinical independence and professional development
Work closely with physicians, advanced practice providers, nurses, care coordinators, behavioral health clinicians, and operational leaders
Collaborate with the COMPASS care-management team and other Seniority Healthcare service lines
Promote continuity and coordination across clinical settings
Participate in interdisciplinary care planning and clinical discussions
Communicate effectively with patients, families, facility staff, and external care partners
Build longitudinal relationships with patients and their families
Help patients and caregivers understand diagnoses, medications, treatment options, and care plans
Support informed decision-making that reflects each patient’s goals and preferences
Promote compassionate, accessible, and dignified care for older adults
Doctor of Medicine or Doctor of Osteopathic Medicine degree
Board certification in Internal Medicine, Family Medicine, Geriatric Medicine, or another relevant primary care specialty
Active, unrestricted Pennsylvania medical license
Willingness to obtain additional state licenses as Seniority Healthcare expands
Active DEA registration, where required for the scope of practice
Experience providing primary care or longitudinal medical management
Experience caring for geriatric, palliative, home-based, or medically complex patient populations
Experience supervising or collaborating with nurse practitioners and physician assistants
Comfort providing care through telemedicine and remote care platforms
Experience working within an interdisciplinary care team
Senior living or home-based primary care experience is preferred
Strong clinical judgment and medical decision-making
Ability to manage an independent primary care panel
Effective and supportive supervisory style
Excellent verbal and written communication skills
Compassionate, patient-centered approach
Ability to work independently while remaining highly collaborative
Proficiency with electronic medical records and virtual care technology
Strong organizational and time-management skills
Adaptability and comfort working in a growing healthcare organization
