Responsibilities
  • Facilitate regular meetings with partners to review performance, address issues and gather feedback.
  • Provide excellent customer service, resolving any customer complaints in a professional manner.
  • Analyze and regularly monitor partner performance data across various metrics like utilization rates, cost per patient, risk adjustment factors and quality measures, generating comprehensive reports to identify trends and areas for improvement.
  • Utilize Medicare reports and claims data, payor reports, 4i CMS portal and other sources as appropriate to track key measures of ACO performance and identify opportunities to improve Provider Partners Connect Care management of Medicare value-based care programs
  • Work with Provider Partners clinical team to provide data analytics and reporting support and assist that team with communication to partners
  • Maintain working knowledge of CMS value-based care plan methodologies and how they impact financial performance of the company and it’s partners
 
Job Requirements
  • Bachelor’s degree in a healthcare or finance related field or equivalent work experience.
  • Strong analytical skills with proficiency in data analysis tools (Excel, Access, Power BI)
  • Excellent verbal and written communication and presentation skills.
  • Ability to work cross-functional with sales, Operations and Revenue Management teams
  • Ability to manage multiple projects simultaneously, track progress and meet deadlines.
