WHAT ARE WE OFFERING?  

This is a unique opportunity to play a key leadership role in improving palliative and end-of-life care for people living in residential aged care in regional South Australia. Working within a collaborative and innovative project team, you will use your advanced clinical expertise via telehealth service delivery to improve palliative and end of life care, build capability within the aged care workforce, and support better outcomes for residents, families and care providers. The role offers the chance to combine clinical leadership, education, mentorship and service development while making a meaningful and lasting impact on the quality of care delivered in Residential Aged Care Facilities.

WHO ARE WE?  

The RSS is a business unit in SA Health that delivers a range of enabling services, such as workforce planning and recruitment, digital health, finance, infrastructure, procurement, and clinical governance support. Our purpose is to strengthen and sustain healthcare delivery in rural and regional areas by providing consistent, efficient, and specialist support functions to the six regional local health networks (LHNs) across South Australia.  We employ around 260 people who work in 37 regional, rural and remote SA locations, or in the RSS's three Adelaide offices.

HOW WILL YOU CONTRIBUTE?  

You will……….  

• Lead the development, delivery and evaluation of nursing and midwifery education programs that support clinical excellence, professional growth and improved patient outcomes.
• Provide support and advice as requested to General Practitioners and Aged Care staff through the facilitation via telehealth of Needs Rounds case conferences regarding symptom management and psychosocial support of residents and their families in Regional SA RACFs
• Conduct Palliative Care Needs rounds with Project RACFs independently as required.
• Provide expert clinical education, advice and support to staff, students and key stakeholders across regional health and aged care services.
• Drive quality improvement, research and evidence-based practice initiatives that strengthen safety, care standards and service delivery.
• Collaborate with clinical leaders and teams to identify learning needs, develop capability and support workforce development priorities.
• Promote contemporary nursing and midwifery practice through leadership, mentoring, competency assessment and the implementation of best practice frameworks.
• Working collaboratively with a Senior Palliative Care medical consultant as part of a multidisciplinary team provide clinical expertise in aged care and palliative care in order to develop evidence-based models of care for Residential Aged Care Facilities (RACF).
• Provide expertise into the development of education frameworks, material and clinical learning objectives for RACF nurses and direct care workers.

• Registered or eligible for registration as a Nurse with the Nursing and Midwifery Board of Australia and who holds, or who is eligible to hold, a current practicing certificate and be Credentialled Nurse Practitioner in Palliative or Aged Care
