PURPOSE STATEMENT:
The National Continuing Care Coordinator oversees the care continuity for patients transitioning from residential behavioral health treatment to outpatient or community-based services. This position ensures seamless transitions, optimizes patient outcomes, and helps maintain long-term recovery through effective coordination with healthcare teams, families, and external providers.
Roles and Responsibilities:
ESSENTIAL FUNCTIONS:
- Develops individualized continuing care plans in collaboration with patients, clinical and outreach teams.
- Works directly with discharge planners to coordinate after care.
- Coordinates the discharge and aftercare process from inpatient or residential programs, ensuring appropriate referrals to outpatient services.
- Communicates regularly with outpatient providers, such as therapists, counselors, or case managers, to ensure the continuity of care.
- Serves as a liaison between different healthcare providers (e.g., primary care, mental health specialists) to ensure the integration of physical and behavioral health services.
- Participates in interdisciplinary team meetings to discuss ongoing patient care, challenges, and interventions.
- Maintains communication with insurance providers and assist in the authorization of continuing care services.
- Maintains detailed reports illustrating the efficacy and performance of our community aftercare partners
- Generates reports for leadership on patient outcomes, specifically readmission rates.
- Ensures all documentation complies with HIPAA and other confidentiality regulations.
- Educates clinical and case management teams on the importance of continuing care and long-term recovery strategies.
- Advocates for patients within the healthcare system, ensuring access to necessary care and services.
- Conducts training sessions for facility teams on best practices in continuing care coordination.
- Communicates and produce remotely through Telehealth and other virtual platforms.
EDUCATION/EXPERIENCE/SKILL REQUIREMENTS:
- High school diploma or GED required. Bachelor’s Degree in a related field preferred.
- Three or more years of experience in a behavioral healthcare setting, with a focus on case management or continuing care coordination required.
- Strong interpersonal and communication skills, with the ability to work effectively with diverse populations.
- Organizational and time management skills for managing a multi-site organization with each facility having their own specific needs around care-coordination when considering regionality & patient populations.
- Knowledge of behavioral health systems, treatment models, and community resources nationally.
- Ability to work collaboratively in a multidisciplinary team environment.
- Crisis management and problem-solving abilities.
LICENSES/DESIGNATIONS/CERTIFICATIONS:
Must have and maintain a valid driver's license and clean driving record and maintain automobile insurance coverage and have access to an automobile.
SUPERVISORY REQUIREMENTS:
Position is an individual contributor.
