 Job Summary

The Licensed Practical Nurse (LPN) will play a crucial role within the Call 4 Health care team, operating in a remote capacity. In this role, the LPN will support Call 4 Health clients by efficiently managing Electronic Health Records (EHR) and patient portals. In addition, the LPN will provide patient education, troubleshoot Remote Patient Monitoring (RPM) equipment and results, counsel patients with chronic diseases, follow up with patients between provider visits, and refill medications in accordance with established protocols and within the LPN scope of practice. The LPN will follow established protocols to facilitate effective communication among healthcare professionals and patients, ultimately enhancing patient outcomes and relieving provider stress.

Duties and Responsibilities 

The following duties and responsibilities reflect the expectations of this position but are not all-inclusive.

  • Monitor physician EHR inboxes and patient portal requests, perform tasks within the LPN scope of practice, and respond to patient inquiries according to established protocols.
  • Process prescription refill requests in accordance with established client protocols and applicable LPN scope of practice; notify providers; ensure patient-care tasks are completed for critical results; and coordinate follow-up care after discharge or surgery.
  • Facilitate referrals and collaborate on prior authorization requests requiring clinical consultation, while also assisting with requests from hospitals, public health departments, nursing homes, funeral homes, and Departments of Labor or Motor Vehicles.
  • Manage Remote Patient Monitoring (RPM) results by tracking patients' real-time disease results and providing education on the management of chronic diseases such as diabetes, hypertension, and
  • Medically screen and qualify patients for clinical trials and perform related follow-up and patient
  • Carry out additional responsibilities as needed to assist the healthcare team and enhance the delivery of patient care.

Education and/or Experience

  • Licensed Practical Nurse certificate
  • Minimum of 2-4 years of full-time clinical experience as an LPN, particularly in primary care for adult, pediatric, and geriatric patients. Experience in OB/GYN, orthopedics, ambulatory care, or home health is also preferred.
  • Experience using at least two electronic health record (EHR) systems is
  • An active, unencumbered LPN license with multistate privileges under the Enhanced Nurse Licensure Compact (eNLC), where Additional noncompact state licensure is considered an asset, and candidates must be willing to obtain additional licenses at the company's request.
