Summary:
The Waiver Case Manager supports individuals with developmental disabilities by connecting them to Indiana's state-funded waiver services. This role involves conducting assessments, developing individualized service plans, coordinating multidisciplinary care, and ensuring compliance with Medicaid and state regulations. Responsibilities:
