Responsibilities

The Health Risk Assessment (HRA) Nurse Supervisor LPN/LVN will assist managers and individuals within their teams to meet performance targets, deliver excellent customer service and achieve desired business objectives while adhering to their individual productivity and health plan reporting requirements. This role will assist and monitor activities within the team as assigned by their manager and escalate as needed.

 

Essential Duties and Responsibilities

  • Maintain 80% of the HRA nurse productivity expectations, adhering to established process, procedures and job requirements defined within their scope
  • Maintain health plan outreach lists, reporting and assigning daily workflows to meet target due dates
  • Complete accurate and timely monthly reconciliation of assessments for billing purposes
  • Identify department training and coaching needs and facilitate retraining sessions to enhance the skills and knowledge of team members as directed
  • Conduct job shadowing, document processes, perform quality monitoring, run and interpret basic reporting
  • Assist manager in resolving patient complaints or complex issues from health plan contact or team members
  • Provide a point of contact for the team when they encounter challenges or need assistance with difficult scenarios
  • Demonstrate effective de-escalation behavior and actions in points of contention with both internal stakeholders and those outside of the organization
  • Identify opportunities for process improvement to enhance outcomes, team efficiency and health plan member satisfaction.
  • Collaborate with managers, teammates and other HarmonyCares leaders to identify and implement change that optimizes the organization
  • Support and encourage teammates by fostering open communication channels as well as role modeling best practice
  • Employees will be required to perform other job-related assignments as requested

Qualifications

Required Knowledge, Skills, and Experience

  • High school diploma or equivalent
  • Current unencumbered State professional nursing license
  • 1+ year of experience as a professional nurse
  • Ability to make sound professional clinical judgment
  • Ability to assess and document patient needs
  • Proficiency in basic clinical skills
  • Proficiency in personal computer use including email, clinical software systems, word processing, and spreadsheet software
  • Must be able to work independently
  • Demonstrated experience with multi-tasking while given competing priorities
  • Ability to communicate in an empathetic, compassionate and professional demeanor at all times
  • Ability to solve problems with demonstrated capacity to embrace complex problems and arrive at effective solutions in a timely manner
  • Ability to analyze problems and resolve issues in a rapid, direct and complete manner
  • Demonstrated short and long-term persistence in meeting objectives and personal development
  • Ability to adapt to new systems and changes as required and apply guidelines/ procedures
  • Excellent communicator with an uplifting and personable manner with outstanding phone etiquette
  • Excellent written communication skills with success in providing notes, updates and written communication via computer systems
  • Excellent data-entry and proven ability to navigate multiple computer screens

Preferred Knowledge, Skills and Experience

  • Associate’s degree or higher
  • 6+ months of experience with HarmonyCares
  • 2+ years of telephonic health risk assessment experience
  • 2+ years of leadership experience and success in exceeding individual or team goals in healthcare or health risk assessments
