The Health Coach will conduct virtual check-ins to track patient progress and support lifestyle, nutrition, and behavior change goals. They will also coordinate with care team members and document interactions within the Electronic Health Record system.

Escalate clinical concerns to licensed providers as needed

Required
1-5 years of experience in health coaching, patient navigation, care coordination, or a similar patient-facing role.
Interest or experience in obesity medicine, lifestyle change, chronic condition support.
Able to efficiently use and adapt to EHRs, telehealth platforms, and a variety of digital communication and workflow tools.
Strong organization skills and comfort managing multiple patient touchpoints.
A strong ability to build trust, motivate patients, and communicate with empathy in a virtual setting.
A reliable internet connection and a private, HIPAA-conscious remote workspace.
Preferred
Health coaching certification and/or a bachelor’s degree in a health-related field
