Health and Wellness Coach PRN

Job Description:

The Health and Wellness Coach is a credentialed healthcare professional responsible for supporting participants in their healthcare journey as they achieve measurable health behavior change. Utilizing evidence-based coaching techniques, the coach guides individuals in setting and reaching specific wellness goals, including lifestyle modification, chronic condition prevention and management.

As a part of the interdisciplinary care team, this role conducts structured coaching sessions, delivering educational resources, and ensuring timely access to health promotion services. The Health and Wellness Coach plays a key role in reducing health risks, improving clinical outcomes, and contributing to overall cost savings for the health system.

Essential Functions

• Reviews pertinent medical history and/or applicable participant information.

• Performs and/or records anthropometrics as applicable to the care recommended by the care team.

• Assess participant needs, readiness for change and self-efficacy while utilizing evidence-based health coaching models and approaches to behavior change to support care team’s guidance.

• Works as part of the interdisciplinary team to support changing health behaviors.

• Engage participants in health coaching by creating strategies and using interventions that are appropriate to the individual considering culture, language, age, and gender.

• Provides education that is within clinical scope and support using adaptive coaching strategies based on the specific needs, learning ability and preferences of the participant.

• Provides full coaching sessions lasting 30-60 minutes live or through digital formats.

• Documents participant activities and coaching/counseling sessions.

• Demonstrates established customer service behaviors and standards.  Treats all clients with courtesy, dignity, respect, and professionalism.

Qualifications

• Must have completed a health coaching program approved by the National Board for Health and Wellness Coaching (NBHWC) certification prior to hire

• Current Health and Wellness Coach Certified or will obtain within 12 months of hire: National Board for Health and Wellness Coaching (NBHWC) certification

• Demonstrated experience in a healthcare or applicable care setting.

Physical Requirements

• Ongoing need for employee to see and read information, labels, assess patient needs, operate monitors, identify equipment and supplies.

• Frequent interactions with patient care providers, patients, and visitors that require employee to verbally communicate as well as hear and understand spoken information, alarms, needs, and issues quickly and accurately, particularly during emergency situations.

• Manual dexterity of hands and fingers to manipulate complex and delicate equipment with precision and accuracy. This includes frequent computer use and typing for documenting patient care, accessing needed information, medication preparation, etc.

• For roles requiring driving: Expected to drive a vehicle which requires sitting, seeing and reading signs, traffic signals, and other vehicles.

The hourly range for this position is listed below. Actual hourly rate dependent upon experience. 

$28.06 - $44.20
