Certified Family Care Coordinator (FCC)
Location: State of Wyoming
POSITION SUMMARY:
The Wyoming Behavioral Institute Outpatient & Telehealth Clinic provides tele-mental health services to youth and their families across the State of Wyoming. A Certified Family Care Coordinator (FCC) serves the role as care coordinator to support access to services for children and families. FCC’s are trained and certified in Magellan’s High Fidelity Wraparound care model and are responsible for comprehensive, strength-based, and culturally responsive support to families with complex needs. FCCs work within the community settings and must be able to effectively collaborate with community members including social service, educational, and government agencies to coordinate the implementation of assessments, crisis plans, and care plans to support children and families. FCCs are responsible for providing accurate and timely communication, providing direct care to students and their families, completing reporting/data entry and documentation with accuracy, and participating in problem-solving and making recommendations to referral sources as well as the treatment team. Other duties as assigned.
ADDITIONAL INFORMATION: Mileage reimbursement for work related traveling activities, work cellphone provided, full time, fully benefited position.
KNOWLEDGE, SKILLS AND EXPERIENCES REQUIRED:
Must be 21 years of age and be a currently active/enrolled Certified Family Care Coordinator. Preferred associate’s degree (or higher) in mental health/behavioral health, Social Sciences, Social Work, or related field, and/or two (2) years of experience in patient care and mental and behavioral health care or equivalent. Experience working in confidential records, completing documentation, and understanding of Magellan timeline and billing processes. Must be able to demonstrate expertise in using Zoom, excel, word, OneDrive, outlook email and calendar, and Magellan’s fidelity program. Demonstrated ability to successfully organize and enter data, complete reporting, organize patient files, communicate effectively through teleservices, and meet licensing, accreditation, and certification standards. Must have knowledge of developmental tasks of children, adolescents, adults, and families and apply knowledge in assessment and planning of care.
WORKING CONDITIONS:
Duties require sitting, standing, and walking regularly. Requires reaching overhead occasionally and lifting occasionally up to 25 lbs. Position requires that the individual be in good physical and mental health sufficient to support behavioral health patients of all ages. FCC must be able to maintain the ability to provide and support services through telehealth, including the need to speak and hear via teleconference. Hours may be irregular including evenings and possibly weekends, duties variable. May be occasional occupational exposure to blood borne pathogens or other potentially infectious materials.
