The referenced pay range represents the minimum and maximum compensation for this job. Individual annual salaries/hourly rates will be set within the job's compensation range, and will be determined by considering factors including, but not limited to, market data, education, experience, qualifications, expertise of the individual, and internal equity considerations.

Responsibilities:

GENERAL PURPOSE:

Serves as a liaison between the organization, patient and their families, where appropriate. Explains procedures and associates costs with patients and potential patients. Communicates the organization's policies and procedures to patients and explains problems, questions and concerns to appropriate staff. Coordinates with proper staff to ensure appropriate patient care is provided.

ESSENTIAL FUNCTIONS

  • Coordinates patient care activities and ensures timely and accurate delivery of services. Acts as the primary point of contact for patients and their families, addressing concerns and facilitating communication between all parties involved. Develops, implements, and monitors individualized treatment plans in collaboration with healthcare providers.
  • Oversees the scheduling and logistics of clinical appointments, procedures, and follow-ups. Manages clinical documentation and ensures accuracy and completeness of patient records. Monitors and evaluates clinical processes to identify areas for improvement and makes recommendations to supervisor for improvements.
  • Works closely with interdisciplinary teams, including physicians, nurses, social workers, and administrative staff, to ensure cohesive patient care. Facilitates effective communication between healthcare providers, patients, and families.
  • Analyzes clinical data and generates reports to track patient outcomes, process efficiency, and other relevant metrics. Utilizes data to inform decision-making.
  • Advocates for patient needs and preferences, ensuring their voice is heard in the treatment planning process. Provides education and support to patients and their families regarding treatment options, procedures, and self-care.
  • Provides guidance, mentorship, and support to lower level coordinators. May conduct training sessions and complete performance evaluations for staff members. May participate in quality improvement initiatives and project management activities.
  • Ensures all clinical activities adhere to relevant healthcare regulations, standards, and best practices. Maintains up-to-date knowledge of regulatory changes and integrates them into practice as needed.
  • Other duties as assigned.


MINIMUM EDUCATION & EXPERIENCE

  • Associate's degree required
  • Or equivalent combination of education and experience
