Chief Medical Information Officer (REMOTE)

The Chief Medical Information Officer (CMIO) serves as the senior clinical informatics advisor to federal clinical and program leadership across the design, build, deployment, and sustainment of a federal health agency's Oracle Health (Cerner) Millennium electronic health record (EHR). In support of an active implementation and deployment program, the CMIO analyzes clinical content and configuration questions and prepares recommendations for federal decision-makers, supports clinical governance and change-control bodies, coordinates clinical participation in testing and data-migration validation, analyzes site readiness criteria, and supports clinical command-center and hypercare operations through go-live and the transition to sustainment. The position pairs deep medical informatics expertise with practical knowledge of EHR build and change-request mechanics and of the federal program environment in which the work is delivered. This role requires a strong background in medical informatics, board certification in clinical informatics, and extensive experience working within federal healthcare environments.

Essential Functions, Responsibilities & Duties may include, but are not limited to:

• Clinical Design, Change Control & Governance: Analyze clinical workflow, content, and configuration questions and prepare functional analyses and decision packages for federal approving officials and the Change Control Board; support the clinical governance bodies that review them and track items to closure once determinations are rendered.
• Deployment Readiness & Go-Live Support: Analyze and report site readiness across interfaces, devices, data migration, training, and clinical workflow to inform federal go/no-go decisions, and support clinical command-center operations through go-live, hypercare, and the transition to sustainment.
• Optimization: Following go-live, maintain and prioritize the clinical optimization backlog for government direction and measure the resulting quality, safety, and efficiency outcomes.
• Patient Safety & Clinical Continuity: Recommend and help operate the EHR patient-safety framework, and draft clinical downtime and continuity-of-operations procedures for federal review and adoption.
• Training & Adoption: Advise on super-user and end-user training strategy, competency standards, personalization events, and post-live proficiency support.
• Stakeholder Engagement & Reporting: Brief federal leadership, site executives, and clinical staff; coordinate with the EHR vendor, systems integrator, and other federal modernization programs; and produce the clinical portions of program deliverables and status reporting.
• Testing, Validation & Data Migration: Coordinate clinical participation in integrated testing and functional validation, recommend acceptance criteria and sampling approaches, and provide clinical subject-matter expertise for legacy-system data migration.
• Team Development: Mentor clinical informaticists and business analysts and support site physician and nursing champions.

Qualifications:

• Education: MD or DO degree required; advanced degree in Health Informatics, Healthcare Administration, or a related field preferred.
• Board Certification: Board certified in Clinical Informatics (ABPM/ABMS) or equivalent health informatics board certification.
• Licensure: Active, unrestricted license to practice medicine in a U.S. state or territory.
• Experience: Minimum of 10 years of clinical experience, with at least 5 years in a leadership role within medical informatics, specifically involving EHR systems design, build, and implementation.
• Implementation & Deployment Experience: At least one full-lifecycle enterprise EHR implementation carried from design and build through go-live and hypercare; multi-site, wave, or cohort deployment experience preferred.
• Oracle Cerner Millennium Experience (Preferred): Hands-on experience with Oracle Health (Cerner) Millennium — its clinical solution set (PowerChart, PowerOrders/PowerPlans, Dynamic Documentation, FirstNet, SurgiNet, PharmNet) and its build, domain, and code-set model.
• Federal Health Experience: Demonstrated experience working within federal health systems, such as the VA, DoD/DHA, IHS, or other federal healthcare agencies; familiarity with federal EHR modernization efforts and joint program office structures preferred.
• Federal Program Delivery: Experience delivering clinical or health IT services in a federal program environment — performance work statements, defined deliverable schedules, integrated master schedules, and a working understanding of which determinations rest with federal officials and how recommendations are packaged for them. Familiarity with FISMA/RMF authority to operate and Section 508 requirements preferred.
• Travel: Ability to travel to deployment sites, including extended on-site support during testing events, go-live, and hypercare windows (estimated 20% annually, concentrated around deployment milestones).

Knowledge, Skills, and Abilities:

• Strategic Vision: Ability to shape and advance a strategic vision for health information technology, aligning recommended EHR initiatives with agency mission and program objectives.
• Interoperability & Data Standards: Working command of HL7 v2, FHIR, C-CDA, and the clinical terminologies (SNOMED CT, LOINC, RxNorm, ICD-10) that govern interfaces, migration, and reporting.
• Change Control & Configuration Management: Fluency in change-control governance — intake and triage, impact and level-of-effort assessment, and delegated versus board-level approval paths — including the discipline that build authorization is effectively build execution in a live configuration.
• Influence Without Positional Authority: Ability to move clinical questions to resolution through analysis, credibility, and consensus-building in an environment where approval authority rests with federal officials, and to accept and execute direction once a determination is made.
• Patient Safety Science: Command of EHR-related safety risk, usability and human-factors principles, SAFER Guides, and safety event analysis applied to configuration decisions.
• Regulatory Knowledge: Comprehensive knowledge of federal health IT regulations, including HIPAA, HITECH, 42 CFR Part 2, ONC certification and information-blocking rules, CMS quality reporting, and applicable accreditation standards.
• Leadership, Communication & Change Management: Ability to communicate complex technical concepts to non-technical stakeholders, brief senior federal executives concisely, and lead adoption and optimization of health information technology with a focus on user engagement and satisfaction.
• Judgment Under Schedule Pressure: Sound clinical judgment applied to framing risk acceptance, mitigation, and escalation options for federal leadership when deployment milestones, build readiness, and clinical safety are in tension.
• Cultural Competence: Ability to work effectively with diverse, rural, and historically underserved patient populations and the clinical teams that serve them.
