You will be responsible for building genuine relationships with clients, coordinating personalized care plans, and managing home care package budgets. Additionally, you will handle client onboarding, maintain regular monitoring calls, and ensure all administrative documentation and KPIs are met.
Do meaningful work, build genuine client relationships and have a manager who genuinely has your back — all while working from the comfort of your Launceston home.
What if your next job gave you the flexibility to work from home, a manager who trusts you to get the job done, and a team that actually supports each other?
We’re looking for an experienced Care Partner to support older Australians across Launceston and surrounding Northern Tasmanian communities, helping them live independently, safely and comfortably in their own homes.
And honestly? This is the kind of job where the manager makes all the difference.
A manager who trusts you
Your manager is approachable, hands‑on and genuinely supportive.
They have an open‑door approach — even when that door is virtual — and want you to feel comfortable asking questions, raising concerns or bringing forward ideas.
They take the time to know their people: how they work, what motivates them, their strengths and where they might need extra support.
You’ll get guidance when you need it and autonomy when you’ve got it.
No micromanaging. No unnecessary checking in. No feeling like someone is constantly looking over your shoulder.
You’ll be trusted to manage your clients, your workload and your day — while knowing your manager is there when you need them.
And when the team needs extra support, they’re not afraid to roll up their sleeves and get involved.
Work from home — and still feel part of a team
Working from home doesn’t mean working alone.
You’ll have the flexibility and convenience of working from your own home in Launceston while being part of a friendly, collaborative team that genuinely helps each other out.
We’re looking for someone who enjoys the independence of working remotely but also knows how to stay connected, communicate and take ownership.
If you’re someone who can organise your own day, manage competing priorities and get things done without needing to be chased — you’ll love this environment.
Clients first. Results matter too.
At the heart of this role are your clients.
You’ll build genuine relationships, understand what matters to each person and coordinate the services they need to live well at home.
You’ll also be responsible for bringing new clients on board — with a target of around two new clients per week — completing the associated paperwork and maintaining regular contact with every client through monthly monitoring calls.
Because great client outcomes and commercial sustainability go hand in hand.
We’re passionate about providing exceptional care, but we’re also a business. Meeting KPIs means we can continue growing, supporting our clients and creating sustainable jobs for our people.
If you’re client‑focused and commercially aware, you’ll fit right in.
What you’ll be doing
You’ll be responsible for:
This role is about care coordination, relationships and client outcomes — not providing hands‑on nursing care.
Who we’re looking for
You might be:
A Certificate IV or higher in aged/community care, care coordination or equivalent experience is required, along with experience in case management, wellness/reablement or aged care service delivery.
