What We’re Looking For

The Care Manager – 1915i is responsible for providing complex care coordination and monitoring to members having a primary Intellectual or Developmental Disability, which may include a secondary mental health or substance abuse, and/or complex medical/physical health need. This position requires a dynamic, proactive approach to assessment, monitoring and complex coordination of care, to ensure quality support and consistent adherence to waiver requirements. The Care Manager – 1915i assists with integrated care management through multi-disciplinary collaboration.

On a typical day, you might:

• Utilize person-centered planning methods & strategies to gather information, to get to know the members supported and to adequately address specific needs of the individual.
• Provide education and support to members on learning about and exercising rights and responsibilities. 
• Complete discovery activities (information gathering and assessment) as a component of the planning process.
• Ensure that assessments and plans are updated as needed and whenever the member’s life circumstances change.
