Position Summary

We are looking for someone who is available to work Monday through Friday, from 11:00 AM to 8:00 PM EST. The RN Care Coach works collaboratively as part of a care management team to guide prospective families through the initial assessment process, assess consumer and caregiver eligibility for services, develop an initial person centric plan of care and meet established timelines for timeliness of assessment and service activation. The RN Care Coach, advocates and educates caregivers to assist with their daily health and well-being needs and that of the consumers. This position supports the ongoing care management needs of cases as needed.


What You Will Do

  • Serves as the initial contact of the coaching team, provides information on services and requirements while guiding the caregiver and consumer through the referral process
  • Educates prospective families 
  • Coordinates and completes all applicable assessments
  • Coordinates consumer service activation and ensures that all required forms and standard procedures are followed for the activation process.
  • Develops strong relationships with case management entities to drive collaboration
  • May provide ongoing coaching to caregiver in conjunction with coaching team as needed to drive effective utilization of established best practices and protocols 
  • Develops, reviews and approves each member’s plan of care on an ongoing basis 
  • Provides clinical consultation, training and education to coaching team 
  • Ensures continuous survey readiness and compliance with all regulatory and accreditation agency standards 
  • Performs other duties as assigned

What You Will Bring

  • Must hold an active Registered Nurse (RN) License in the state(s) of employment
  • Minimum two (2) years of recent experience in direct care of elders or adults with disabilities
  • Experience in case management, care planning and assessment for cases including but not limited to eldercare, people with disabilities, and other complex medical needs in a health care setting 
  • Proficiency in common software and web-based applications (e.g. MS Office, case management systems) 
  • Additional state-specific qualifications may be required.
  • Experience in the Emergency Room (ER) and triage is preferred
  • Fluency in both English and Spanish is required
  • Experience working with individuals living with dementia or other memory-related conditions preferred.

The pay range for this position is $65,116 - $79,744. The actual wage offered may be lower or higher depending on budget and candidate experience, knowledge, skills, qualifications, and geographic location.
