And we are growing! The Behavioral Health Team at OCES is seeking two Behavioral Health Clinicians to support our consumers in the community using a person-centered approach to address complex medical and behavioral health needs.

 

Job Duties/Responsibilities:

• Work directly with older adults with complex behavioral health conditions who require
psychotherapy to reduce risk/isolation and improve overall wellbeing.
• Complete assessments and develop comprehensive, person-centered care plans with
consumers, caregivers, and families.
• Provide advocacy, support, and assistance to address unmet needs.
• Provide crisis intervention services, as needed.
• Maintain and enhance knowledge of local resources and programs, both mental health-related
and otherwise, and make referrals as needed.
• Conduct outreach to local providers about Behavioral Health services.
• Coordinate and/or facilitate support groups based on community need.
• Collaborate with and consult with other OCES programs to support behavioral health needs of
consumers.
• Maintain timely and appropriate case documentation according to state regulations, agency
standards, other funding sources/grant requirements.
• Other duties as assigned.
Required Knowledge/Skills/Abilities:
• Excellent verbal and written communication skills.
• Excellent organizational skills and attention to detail
• Proficient with Microsoft 365 and related software


Education/Experience:

• Master's degree in social work or related field
• Current LICSW or LCSW license
• A minimum of 3 years of experience.


Work Environment:

• Prolonged periods of sitting at a desk and working on a computer.
• Must be able to lift up to 25 pounds at times
• Must have personal computer with secured internet access at home
