Admissions Care Coordinator (Customer Service Associate Analyst)- Evernorth
Overview
Join a team that is redefining specialty pharmacy and infusion care. As an Admissions Care Coordinator, you will play a key role in helping patients start therapy with confidence. You will guide patients through onboarding, remove financial barriers, and ensure a smooth transition into care.

Responsibilities
• Coordinate patient onboarding from referral through therapy start, ensuring a timely and seamless experience.
• Verify insurance coverage and clearly communicate financial responsibility to patients with empathy and clarity.
• Identify, secure, and manage financial assistance and copay support to reduce access barriers.
• Maintain accurate and complete patient records across all stages of care.
• Partner with cross-functional teams to ensure care plans are executed effectively.
• Serve as a trusted point of contact for providers, payers, and referral sources.
• Monitor workflows and identify opportunities to improve efficiency and patient satisfaction.
• Support onboarding and mentoring of new team members.
• Help uphold and enhance policies and procedures.
• Address and resolve patient or partner concerns promptly.
Qualifications
Required:
• High School Diploma or GED
• Minimum of 1+ years of experience in healthcare, customer service, contact center, or related field
• Strong communication skills
• Proficiency with Microsoft Office
• Ability to manage multiple priorities
• Strong organizational and problem-solving skills
• Commitment to teamwork and patient experience
Preferred:
• Experience in pharmacy, home health, or home infusion
• Knowledge of medical terminology and insurance verification
• Experience supporting financial assistance programs
• Prior mentoring experience
