The Field Relations Coordinator manages team-based population health by developing relationships with providers and members while coordinating care plans. They also perform screenings, facilitate linkages to community resources, and monitor service outcomes to ensure member needs are met.

The Social Worker will assess, plan, and monitor comprehensive care plans for high-risk health plan members to ensure optimal health outcomes. They will coordinate services across the continuum of care and support members and their families in coping with illness.
