The Clinical Nurse Consultant will provide high-quality clinical assessment, care planning, and intervention for heart failure patients within the Hearts at Home program. They will leverage virtual care and remote monitoring technology to coordinate care and support patients in self-management.

Members will provide lived experience perspectives to influence the design, delivery, and continuous improvement of the Medicare Mental Health Check In service. They will participate in quarterly meetings, co-design workshops, and policy reviews to ensure services remain safe, inclusive, and person-centred.

The role involves assigning accurate clinical codes to patient episodes of care using ICD10AM and Australian Coding Standards. It also requires participating in ongoing training, education sessions, and quality audits.
