Deliver high-quality, evidence-based physiotherapy assessments and exercise prescriptions for heart failure patients via virtual platforms. Collaborate with multidisciplinary teams to support early hospital discharge and contribute to the ongoing development of the Hearts at Home program.
The practitioner will deliver evidence-based, culturally safe mental health interventions via telephone and digital platforms for individuals with mild to moderate mental health concerns. They will also conduct comprehensive assessments, develop treatment plans, and maintain accurate clinical documentation within a multidisciplinary team.
Members will provide lived experience perspectives to influence the design, delivery, and continuous improvement of the Medicare Mental Health Check In service. They will participate in quarterly meetings, co-design workshops, and policy reviews to ensure services remain safe, inclusive, and person-centred.
