The Care Manager will guide patients through a 3-month virtual cardiac rehabilitation program using 1:1 video calls and group education sessions. They are responsible for developing personalized treatment plans, monitoring patient progress, and coordinating care with Medical Directors and referring providers.

The Director of Customer Success will manage the full post-sale lifecycle, including onboarding, referral growth, and renewals to drive revenue. They will also serve as a cross-functional intelligence hub, translating field insights into actionable strategies for payer contracting, marketing, and operations.

The Member Engagement Lead will manage patient panels to ensure program adherence, appointment attendance, and treatment completion. They will also build and document outreach protocols while serving as the primary human connection for cardiac patients throughout their recovery.
