Infusion Case Manager RN
The Infusion RN conducts medical reviews to determine eligibility for home infusion therapy and coordinates the transition of care for patients from facility-based settings to in-home care. They also educate patients on the infusion process and monitor ongoing therapy tolerance to ensure safe and cost-effective outcomes.
