Telephonic Triage Nurse
Provide telephone triage and nursing support to hospice patients and families, assessing symptoms, prioritizing urgent needs, and offering care guidance or arranging an in-person visit. Coordinate with on-call staff and the interdisciplinary team, document calls in electronic medical records, and support patient and family choices and quality of life.
