I’m a healthcare professional with experience in hospital case management and skilled nursing facility social work. My background includes discharge planning, care coordination, and working with patients, families, and healthcare teams to address complex needs. In my current role, I manage a caseload of around 80 residents and work closely with physicians, nurses, therapists, and families to coordinate safe discharges and resolve placement challenges. Managing a high caseload has taught me how to prioritize competing demands, stay organized, communicate effectively, and handle multiple tasks at once. I’m experienced in handling sensitive patient information in accordance with HIPAA requirements, documenting in EMR systems such as Epic, and coordinating with healthcare providers and community resources to resolve challenges and find solutions. I’m now looking for a fully remote position in healthcare operations, care coordination, patient services, administration, or healthcare recruiting where I can apply my experience, continue learning, and grow professionally.
