What You'll Own:
- Carry a direct virtual panel of adults living with serious mental illness, providing longitudinal psychiatric care including diagnostic evaluation, psychotropic medication management, and crisis stabilization
- Serve as the clinical psychiatry resource for Constella's NP pod, available during scheduled weekly blocks for warm hand-offs, curbside consultation, and real-time case review when complex behavioral health needs arise mid-visit
- Collaborate with NPs on psychotropic regimen design, especially for members with co-occurring conditions, treatment resistance, or high medication complexity
- Leverage Constella's on-the-ground team including NPs, MAs, and CHWs to ensure the clinical groundwork is done before and between your visits: labs ordered and resulted, symptom checks completed, medication reconciliation confirmed, and key collateral gathered so your virtual time is used at the top of your license
- Partner with CHWs, Social Workers, and MAs as part of an integrated care team, understanding that psychiatric stabilization rarely happens in isolation from housing, substances, and social context
- Coordinate with inpatient psychiatric facilities, crisis services, and community providers to manage transitions of care and prevent unnecessary hospitalization
- Contribute to collaborative practice agreements and provide the supervisory structure NPs need to work at the top of their license
- Document clinical work thoroughly and on time in our EHR
- Help shape the psychiatric model itself. Your clinical judgment and feedback will directly influence how Constella builds and scales its behavioral health infrastructure in Kentucky
Who We Need:
- A board-certified psychiatrist (MD or DO) licensed in Kentucky or willing to obtain licensure promptly, with experience treating adults with serious mental illness (schizophrenia spectrum, bipolar disorder, treatment-resistant depression, and co-occurring SUD)
- Someone comfortable in a collaborative, NP-forward care model and able to serve as a trusted clinical anchor. A clinician who has worked in community mental health, CCBHC, FQHC, or safety-net settings and understands what psychiatric care looks like outside private practice
- Available for consistent warm hand-off windows each week (specific blocks to be determined collaboratively, but reliability during those windows is essential)
- Comfortable with virtual care as a primary modality, and realistic about both its strengths and its limits for this population
- Someone who gives and receives feedback easily, and who wants to help build a model rather than inherit one
Logistics & Structure:
- Location: Fully virtual; Kentucky licensure required or willing to obtain promptly (Constella will reimburse licensing fees)
- Starting commitment: Part-time, with specific hours and session structure determined together; path to full-time as Constella's Kentucky panel grows
- Warm hand-off availability: A few defined blocks per week, scheduled in advance and agreed upon mutually
- Compensation: $275 - $300 per hour
What Success Looks Like:
- A stable virtual panel of members receiving consistent, high-quality psychiatric care
- NPs who feel clinically supported and confident managing behavioral health complexity between consults
- Reduced psychiatric crisis episodes, avoidable ED visits, and inpatient admissions over time
- Strong relationships with your pod, with community psychiatric resources, and with inpatient partners managing transitions
- Clinical insight that materially shapes how Constella delivers and scales psychiatric care in Kentucky
Your Superpowers:
- Board certification in psychiatry; MD or DO
- Active Kentucky medical license or willingness to obtain promptly; Constella will reimburse licensing fees
- 3+ years of post-training experience treating adults with SMI, preferably in a community or safety-net setting
- Experience with or strong interest in collaborative care models and NP supervision
- Comfort with psychopharmacology across a complex, high-acuity population including SUD, metabolic comorbidities, and polypharmacy
- Strong EHR and documentation habits
- A collaborative, non-hierarchical disposition
