Alice's Place, LLC is seeking a dependable, compassionate, and professional Assessment Nurse to join our Structured Family Caregiving (SFC) team.
Position Responsibilities

The Assessment Nurse will:

Complete initial and ongoing nursing assessments for Structured Family Caregiving participants
Conduct in-home visits as assigned
Assess participants' health status, functional needs, medications, and overall care needs
Complete required nursing documentation accurately and on time
Identify changes in condition and communicate concerns to the appropriate care team
Provide caregiver education and support when needed
Maintain participant confidentiality and follow HIPAA requirements
Follow Alice's Place policies and applicable Medicaid/program requirements
Travel to assigned participant homes throughout designated service areas

Qualifications

Current Georgia nursing license
RN or LPN with appropriate experience and qualifications for assigned assessments
Experience working with the elderly and/or individuals with disabilities preferred
Strong assessment and documentation skills
Dependable transportation
Valid driver's license and automobile insurance
Excellent communication and organizational skills
Must be dependable and able to complete documentation within required timeframes
Home health, Medicaid waiver, community-based services, or long-term care experience is a plus
