As a Senior Care Coach supporting our Illinois HealthChoice Medicaid program, you'll play a dual-impact role: helping Medicaid members achieve better health outcomes while mentoring and supporting a team of Care Coordinators who make that care possible.
You will serve as a trusted resource, providing coaching, training, and guidance to team members, supporting onboarding and development efforts, and helping ensure member-centered care. You'll collaborate with interdisciplinary teams, monitor member progress, connect individuals and families with valuable community resources, and help drive positive outcomes through proactive care planning and advocacy.
Upon hire, you will be assigned to support one of the following specialty areas: Maternity, Pediatrics, Physical Health, or Behavioral Health.
This is an exciting opportunity for someone who is passionate about leadership, collaboration, and improving the health and well-being of Illinois Medicaid members.

  • Provide coverage and support for Care Coordinators and be able to document in the clinical system.
  • Mentor and support the team, as well as training initiatives.
  • Support new hire onboarding including but not limited to precepting and case audits.
  • Provide daily reports to the Care Coordinators regarding outstanding cases and review daily compliance dashboards.
  • Support audit initiatives and support any internal and external auditing processes for the team.
  • Complete field work support and review reports, as needed.
  • Coordinate, monitor, and evaluate the case management plan against the member's personal goals.
  • Ensure the member is progressing towards desired outcomes by continuously monitoring care through use of assessment, data, conversations with members, and active care planning.
  • Guides members/families towards resources appropriate for their care. Services are driven by facilitating interactions with other payer sources, providers, interdisciplinary teams, and others involved in the member's care as appropriate and required by our comprehensive contract.

Required Qualifications

The Senior Care Coach must meet one (1) of the following requirements:

  • Bachelor's degree in social sciences, social work, human services, or a related field.
  • Unrestricted Licensed Practical Nurse (LPN) in the state of Illinois with one (1) year experience conducting comprehensive assessments and provision of formal service for the elderly.
  • Licensed Professional Counselor (LPC).
  • Licensed Social Worker (LSW).

The Senior Care Coach must meet all following requirements:

  • Must reside in Illinois or a border state within a commutable distance of IL.
  • 3+ years of experience in health care and/or case management.
  • 2+ years of experience with Medicare & Medicaid recipients and/or long-term care and/or Home and Community based services and/or working with managed care organizations.
  • Ability to use a variety of electronic information applications/software programs including electronic medical records.

Preferred Qualifications

  • Master's degree in social sciences, social work, human services, or a related field.
  • CCM certification.
  • Experience with health promotion, coaching, and wellness.
  • Expertise of community health and social service agencies and additional community resources

Additional Information

  • Workstyle: This is a remote position that requires travel.
  • Travel: Up to 25% field-based interactions conducting care team oversight visits, meeting with members and/or their families, community partners and other care teams. May need to attend occasional onsite meetings in Humana's Illinois locations.
  • Typical Workdays and Hours: Operating hours are Monday - Friday 8:00 AM - 7:00 PM CST but core hours for this position are primarily Monday – Friday, 8:00 AM – 5:00 PM CST. May need to be flexible with work schedule based on business needs.

Driving

This role is part of Humana's driver safety program and therefore requires an individual to have a valid state driver's license and are expected to maintain personal vehicle liability insurance. Individual must carry vehicle insurance in accordance with their residing state minimum required limits, or $25,000 bodily injury per person/$25,000 bodily injury per event /$10,000 for property damage or whichever is higher.

TB

This role is considered patient facing and is part of Humana's Tuberculosis (TB) screening program. If selected for this role, you will be required to be screened for TB.

Work at Home Requirements: To ensure Home or Hybrid Home/Office employees’ ability to work effectively, the self-provided internet service of Home or Hybrid Home/Office employees must meet the following criteria: At minimum, a download speed of 25 Mbps and an upload speed of 10 Mbps is required; wireless, wired cable or DSL connection is suggested. In certain roles, the minimum recommended internet speed required by Humana may not be sufficient for business needs. Humana reserves the right to require associates to upgrade their internet service if necessary. Work from a dedicated space lacking ongoing interruptions to protect member PHI / HIPAA information.

 

Travel: While this is a remote position, occasional travel to Humana's offices for training or meetings may be required.

Pay Range

The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.


 

$65,000 - $88,600 per year


 

This job is eligible for a bonus incentive plan. This incentive opportunity is based upon company and/or individual performance.
