RN Intake Hospital Liaison

Job Description:

The RN Intake Hospital Liaison is a Registered Professional Nurse responsible for coordinating the intake process for patients referred to home care services. Serving as the primary liaison between hospitals, referral sources, patients, families, and internal teams, this role ensures a seamless transition from hospital to home. Responsibilities include reviewing referrals, coordinating appropriate home care services, facilitating timely admissions, and providing exceptional customer service by addressing patient inquiries and concerns with professionalism, compassion, and efficiency.

Requirements:

• Current NYS RN License
• Allscripts a MUST
• CHHA experience a MUST
• Computer literacy; experience with Home Care Home base a MUST
• Excellent communication skills
• Highly organized, possess a strong attention to detail
• Experience with multitasking and working in a high pressure work environment.
