Job Summary:
TheraCare Home Health is seeking a detail-oriented and organized Home Health Scheduler (LVN Preferred) to manage patient and clinician schedules across multiple locations. This role is responsible for maintaining accurate scheduling, coordinating staffing needs, and managing day-to-day schedule changes with field staff and patients. Additional responsibilities include generating and reviewing reports, monitoring visit utilization and staff productivity, and supporting overall patient care coordination and safety.
QualificationsQualifications:
High School Graduate (college preferred)
Licensed Vocational Nurse, per the state requirements.
Prior scheduling experience preferred with multiple disciplines
Good computer skills including but not limited to MS Office, MS Excel, Outlook
Experience using HomeCare HomeBase preferred
Knowledge of basic medical terminology
Ability to articulate interpretation of reports
Excellent customer services skills, both internal and external
Well organized with the proven ability to be efficient while multi tasking
Essential Functions:
Understands and adheres to established TheraCare Home Health policies and procedures
Maintains accurate scheduling for clients and staff for multiple disciplines
Contacts care providers and clients regarding day-to-day changes
Provides staffing for sick leave, vacation, long term leave
Produces reports and articulates interpretation to the administrator
First line in visit utilization, missed visits and employee productivity
Discerns client services required as outlined in agreements, urgent requests and care plans
Responsible for assuring patient/resident safety.
Performs other duties as assigned
