The Project Manager will lead the planning, implementation, and ongoing management of the Community Health Worker Pilot program in New Jersey. This role involves coordinating cross-functional teams, monitoring performance metrics, and ensuring compliance with Medicaid and state requirements.

The Project Manager will support the planning, implementation, and ongoing management of the Community Health Worker (CHW) Pilot, a state-approved and funded initiative under the NJ FamilyCare program. This role is responsible for ensuring successful execution of the pilot, which addresses critical needs including high prevalence of Type 2 diabetes, elevated social determinants of health (SDoH) risk, and disproportionate utilization among Medicaid members in targeted counties.
The Project Manager will lead cross-functional coordination, operational planning, and performance monitoring to ensure the pilot achieves its defined goals, including improving member engagement, enhancing self-management, increasing primary care utilization, and reducing emergency department and inpatient utilization.
Lead pilot implementation across all phases, including planning, execution, monitoring, and evaluation.
Develop and manage project plans, timelines, and key milestones aligned to state requirements and program design.
Coordinate cross-functional teams including care management, behavioral health, pharmacy, providers, community partners, and external stakeholders.
Ensure alignment of staffing, workflows, training, and operational readiness for pilot launch and execution.
Monitor pilot performance metrics, including engagement rates, utilization outcomes, quality measures, and SDoH interventions.
Identify and manage risks, issues, and dependencies; implement mitigation strategies to maintain timelines and outcomes.
Oversee reporting requirements, including internal reporting and external reporting to state partners and evaluators.
Collaborate with external evaluators (e.g., Rutgers Center for State Health Policy) to support data collection, validation, and analysis.
Ensure compliance with Medicaid, DMAHS, and contractual requirements.
Facilitate governance, including status meetings, stakeholder updates, and decision-making forums.
Support development and refinement of workflows, documentation standards, and operational processes.
Partner with leadership to support scalability planning and long-term sustainability of the pilot.
3–5 years of project management experience in healthcare, Medicaid, or population health programs.
Ability to travel up to 10% throughout New Jersey
Experience leading complex, cross-functional initiatives with multiple stakeholders.
Strong understanding of healthcare operations, care management, or community-based interventions.
Experience managing timelines, deliverables, risks, and performance metrics.
Strong communication, organizational, and analytical skills.
Project Management certification (PMP or equivalent) preferred but not required.
Valid driver’s license required; may require onsite or community-based engagement.
Experience supporting Medicaid programs or NJ state initiatives (DMAHS familiarity preferred).
Experience with CHW programs, SDoH initiatives, or chronic disease management (e.g., diabetes).
Experience with program implementation, evaluation frameworks, or pilot design.
Familiarity with care management platforms, reporting systems, and healthcare data analytics.
Experience working with external partners, including academic evaluators or state agencies.
Bachelor’s degree in healthcare administration, Public Health, Business, or related field or equivalent work experience; Master’s degree preferred.

The typical pay range for this role is:
$66,330.00 - $145,860.00This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.
