Role Summary

As a PRN Social Worker supporting home health, you will help clients and families navigate psychosocial needs and community resources to promote safe, appropriate care planning—while working fully remote. Key responsibilities include completing psychosocial assessments, identifying barriers to care, coordinating referrals to community services, supporting advance care planning discussions as appropriate, and documenting thoroughly within required timeframes.

Required qualifications/skills:
- BSW or MSW from an accredited program
- Current Arizona social work licensure (as applicable)
- Home health/hospice or medical social work experience preferred
- Strong clinical judgment, communication, and documentation skills
- Ability to manage PRN caseload needs with reliability in a remote setting

How Your Day Flows

You’ll begin by reviewing new referrals and messages, then plan outreach to clients, caregivers, and partner agencies. Throughout the day, you’ll move between scheduled phone/video check-ins, timely follow-ups, and coordination touchpoints that keep plans moving forward. You’ll prioritize urgent needs, adjust your schedule as new information comes in, and close out documentation so records remain accurate and audit-ready. By day’s end, you’ll have clarified next steps for multiple households and ensured handoffs are clear for the broader care team.
