Work Setting: On the Road for Patient Visits & Training at the Local Office or Remotely

Responsibilities:
Deliver physical therapy services in accordance with the physician’s plan of care and state Physical Therapy Practice Act.
Assess and evaluate patients’ functional status, including therapeutic, rehabilitative, and mobility needs.
Develop and implement individualized physical therapy programs focused on rehabilitation and functional improvement.
Evaluate home environments for safety and accessibility to support patient independence.
Assess muscle strength, mobility, gait, and range of motion to determine rehabilitation potential.
Educate patients and caregivers on the proper use and care of therapeutic appliances and assistive devices.
Prioritize therapy needs and adjust treatment plans as necessary.
Monitor and report patient responses to therapy and any changes in condition to the physician.
Provide instruction and training to family/caregivers and other personnel on therapy techniques and goals.
Participate in case conferences to communicate therapy objectives and progress.
Train patients in the use of prosthetic devices when indicated.
Identify additional home health needs and coordinate referrals as appropriate.
Prepare and submit accurate clinical documentation, including progress summaries and discharge plans, per Summit policy.
Offer physical therapy consultations to patients and caregivers as needed.
Conduct in-service education programs for staff and participate in peer consultations.
Supervise Physical Therapy Assistants (PTAs) in compliance with state regulations and organizational policies.
Evaluate home health aide care every two weeks when therapy is the only skilled service.
Collaborate with interdisciplinary team members to ensure comprehensive, patient-centered care.
Qualifications
Qualifications:
Bachelor’s, master's or doctorate in physical therapy from an accredited program.
Active license to practice as a physical therapist in the state of Indiana.
2 years of previous clinical experience as a physical therapist, preferred.
Prior home health or community-based experience is preferred but not required.
Strong verbal and written communication skills, excellent organizational abilities, and the capacity to work independently and as part of a team.
