As a Physician Designee, you’ll provide physician support to the hospice program by partnering with the Hospice Medical Director and interdisciplinary team to ensure high-quality, compliance, and patient-centered care. You’ll support patients and families during deeply meaningful moments and assist with clinical decision-making that honors each patient’s goals and values. When designated by the Hospice Medical Director, you may act on their behalf to support continuity of care, regulatory compliance, and timely medical oversight for hospice patients.

As a Physician Designee, you will: 

  • Support CMS & Regulatory Compliance – Assist the Hospice Medical Director in meeting responsibilities under the CMS Hospice Conditions of Participation, ensuring care practices align with federal, state, and local regulatory requirements.
  • Assist with Eligibility & Recertification – Review hospice eligibility as delegated, support admission determinations, and complete Certifications of Terminal Illness (CTIs), recertifications, and required face-to-face (F2F) encounters when designated by the Hospice Medical Director.
  • Provide Clinical Consultation – Serve as a physician resource to the interdisciplinary team to support symptom management, disease progression discussions, and complex medical decision-making related to end-of-life care.
  • Support Interdisciplinary Collaboration – Work collaboratively with hospice clinicians, attending physicians, patients, families, and referral sources to promoted coordinated, compassionate, and patient-centered care.
  • Participate in Quality Care & Program Improvement – Support Quality Assessment and Performance Improvement (QAPI) initiatives and contribute to maintaining high clinical standards across the hospice program.
  • Provide Coverage as Needed – Provide physician coverage for the Hospice Medical Director when designated, helping ensure continuity of care and timely clinical decision-making.
  • Other Duties as Assigned – The duties and responsibilities listed above are representative of the nature and level of work assigned and are not necessarily all inclusive. 

This job also requires: 

  • Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO).
  • Current, unrestricted medical license in the state of practice (MN and ND)
  • Board certified or board eligible in Family Medicine, Internal Medicine, Geriatrics or related specialty.
  • Hospice or Palliative Medicine board certification preferred.
  • Demonstrated knowledge of CMS Hospice Conditions of Participation.
  • Strong expertise in symptom management and end-of-life care.
  • Excellent communication and collaboration skills.
  • Ability to pass state mandated background checks.
  • Physical capability to perform all essential job functions.
  • Ability to read, write, and speak English to ensure effective communication with team members, clients, and families.
