The RN Care Manager assists enrollees to access necessary covered benefits identified in the care plan and provide referral and coordination of other services in support of the care plan. The Care Manager is the primary contact person for members and is a member of a Care Management Team that assures that the members’ needs are met. The Care Manager assists enrollees to obtain needed medical, social, educational, psychosocial, financial, and other services in support of the care plan irrespective of whether the services needed are covered benefits.

 Primary Responsibilities:

• Assess, plan and implement care strategies that are individualized by member and directed toward the most appropriate, lease restrictive level of care.
• Utilize the UAS-NY to manage the Person Centered Service Plan that meets the member's needs
• Manage the person centered service plan throughout the continuum of care as a single point of contact
• Communicate with all stakeholders the required health-related information to ensure quality coordinated care and services are provided expeditiously to all members
• Advocate for members and their families as needed to ensure the member’s needs and choices are fully represented and supported by the health care team

Required Qualifications:

• Associate’s degree 
• Current, unrestricted RN license for the state of New York
• 4+ years of experience working within the community health setting in a health care role
• 2+ year of experience in long-term care, home health, hospice, public health or assisted living
• 1+ years of relevant community case management experience coordinating care for individuals with complex needs
• Must hold active New York state ID

Preferred Qualifications:

• A background in managing populations with complex medical or behavioral needs
• Experience with electronic charting
• Experience with arranging community resources

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). The hourly pay for this role will range from $40.00 to $54.00 per hour based on full-time employment. We comply with all minimum wage laws as applicable.
