What You'll Do

• Evaluate patients in their homes and assess functional abilities and individual needs.
• Develop individualized treatment plans and measurable goals.
• Provide therapeutic interventions to improve activities of daily living, safety, and independence.
• Educate patients and caregivers on home exercises, adaptive techniques, and fall prevention.
• Monitor patient progress and adjust treatment plans as clinically appropriate.
• Complete accurate, timely, and compliant clinical documentation.
• Collaborate with the interdisciplinary care team to support positive patient outcomes.

Qualifications

• Graduate of an accredited Occupational Therapy program.
• Active Florida Occupational Therapist license.
• Current CPR certification.
• Required AHCA Level II background screening.
• Home health experience preferred.
• Strong clinical judgment, communication, and organizational skills.
• Ability to work independently and manage field visits effectively.
• Valid driver's license, current auto insurance, and reliable transportation.
