In this Care Manager role, will be an essential element of an Integrated Care Model by relaying the pertinent information about the member needs and advocating for the best possible care available, and ensuring they have the right services to meet their needs.
 

 

If you reside in North Carolina, you will have the flexibility to work remotely* as you take on some tough challenges

 

Primary Responsibilities: 

  • Assess, plan and implement care management interventions that are individualized for each member and directed toward the most appropriate, least restrictive level of care 
  • Identify and initiate referrals for both healthcare and community-based services; including but not limited to financial, psychosocial, community and state supportive services 
  • Develop and implement care plan interventions throughout the continuum of care as a single point of contact 
  • Communicate with all stakeholders the required health-related information to ensure quality coordinated care and services are provided expeditiously to all members 
  • Advocate for members and families as needed to ensure the member’s needs and choices are fully represented and supported by the health care team
  • Ability to provide field-based visits to members locally (up to 25%25), up to 120 miles round trip (60 miles one way) (includes mileage reimbursement)

Required Qualifications: 

  • Unrestricted RN license in the state of North Carolina
  • Must be a resident of NC
  • 4+ years of experience working within the pediatric/maternity community setting or 2+ years of experience providing community-based care management to pediatric/maternity members
  • The ability to travel in assigned region to visit Medicaid members in their homes and/or other settings, including community centers or providers' offices 
  • Intermediate skills in Microsoft Office Suite 
  • Willing to work with both pediatric and maternity populations 
  • Driver’s License and access to a reliable transportation
     


Preferred Qualifications:  

  • Bachelor of Science in Nursing (BSN)
  • CCM certification
  • Experience in managing populations with complex medical or behavioral needs 
  • Experience working with both maternity and pediatric populations
  • Experience working with members in community and home-based settings
  • Fluent in Spanish

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). The hourly pay for this role will range from $29.00 to $52.00 per hour based on full-time employment. We comply with all minimum wage laws as applicable.
