OPTIONS FOR REMOTE

The LVN/LPN Bridge Navigator supports patients and families as they transition from referral to hospice admission and through the early stages of hospice care. Serving as a consistent point of contact, the Bridge Navigator helps remove barriers, coordinates communication among referral sources and care teams, reinforces education, and promotes timely access to services.

The majority of the time is devoted to extensive phone calls to patients and physicians to obtain orders.  Could be assigned other tasks not listed. 

The position does not independently certify terminal illness, determine hospice eligibility, complete duties legally reserved for an RN, prescribe treatment, or independently develop or modify the hospice plan of care.

If remote, you must be able to provide reliable internet service to perform daily functions via the internet/Wi-Fi.

Minimum Qualifications

  • Current, unrestricted LVN or LPN license in the state of practice. (COMPACT LICENSE IS A MUST)
  • Graduate of an accredited practical or vocational nursing program.
  • Current CPR certification, if required by organizational policy.
  • At least [one year] of clinical nursing experience.
  • Valid driver’s license, automobile insurance, and reliable transportation when travel is required.
  • Ability to meet all background screening, health, vaccination, and competency requirements.

Preferred Qualifications

  • Hospice, palliative care, home health, oncology, geriatrics, hospital discharge, care-transition, or case-management experience.
  • Experience educating patients and caregivers during serious illness.
  • Familiarity with Medicare hospice services and interdisciplinary care.
  • Experience using electronic medical records and referral-management systems.
