Our Licensed mental health (MH) professional works as part of a multidisciplinary team to provide the highest quality of care to our patients. The MH professional engages in identification, support and resource management regarding various sources of diabetes distress. The MH professional will provide patient-facing services (1:1 with potential for group) related to the patient's diabetes distress. They will also recommend referrals to local services as their service does not provide long-term or high-acuity care beyond the scope of diabetes distress. They will also act as primary support to the care team for guidance on any urgent MH escalations that may arise during the patient journey. 

This is a full time, remote, home-office based role eligible for a full benefits package including paid holidays, PTO, and competitive pay in a mission-based work environment. A private, quiet, home office space is required. The schedule for this role is Monday-Friday from 11am-8pm Eastern. 

This role is open to candidates residing anywhere in the United States who are legally authorized to work in the U.S. on a full-time basis, now and in the future, without employer sponsorship.
  • Master’s Degree in Social Work, Counseling or related field from an accredited university required. 
  • We are seeking a licensed mental health professional, diverse credentials and licensure types are welcome, must be able to independently practice, and license must be in good standing.
  • Currently holds a New York license or is able to obtain a New York license
  • Monitor behavioral health escalation channel; triage and escalate high-risk cases to ensure timely, appropriate intervention. 
  • Minimum of  five years of post master’s clinical experience as a licensed therapist required. 
  • Prior experience working with under-resourced populations required, preferably experience working with adults with T1D. 
  • Fluency in English required. Spanish Fluency preferred.
  • Strong interpersonal and communication skills and the ability to work effectively with people of diverse cultures, ages, and economic backgrounds in a culturally competent and sensitive manner. 
  • Ability to provide high quality care in a virtual environment.
  • Ability to listen actively, communicate with empathy and gather information in a respectful manner. 
  • Ability to use independent judgment and analyze information from multiple sources. 
  • Ability to communicate information to medical professionals, clinical employees, coaches, insurance companies, and other community organizations over the phone or virtually.

As a part of our care team, this role is expected to get licensed in states where we provide services. Costs associated with licensing and licensing support is provided by Blue Circle Health. For this role, being licensed in New York state or be able to get licensed. Below is a summary of New York state endorsement requirements. 

New York LCSW by endorsement 
If an applicant is Licensed in another State, we will apply by Endorsement.
Requirements: An applicant for LCSW who was licensed as a clinical social worker in another jurisdiction and has at least 10 years of practice in the 15 years prior to application. The initial license must have been issued on the basis of meeting requirements similar to those in New York State, including:
  • MSW degree with clinical content;
  • Post-MSW supervised experience in diagnosis, psychotherapy and assessment-based treatment planning, and;
  • ASWB clinical social work examination.
As an LCSW, to obtain licensure in the state of New York, you must be able to provide a list of supervisors who will verify at least 10 years of practice in clinical social work in the 15 years prior to application for licensure by endorsement. 

The applicant MUST be able to provide and help us with the completion of the following:
  • A list of supervisors who will verify at least 10 years of practice in clinical social work in the 15 years prior to application for licensure by endorsement. The supervisors must be able to attest to the following about the applicant when asked to complete the work experience form: “The practice of clinical social work encompasses the scope of practice of licensed master social work and, in addition, includes the diagnosis of mental, emotional, behavioral, addictive and developmental disorders and disabilities and of the psychosocial aspects of illness, injury, disability and impairment undertaken within a psychosocial framework; administration and interpretation of tests and measures of psychosocial functioning; development and implementation of appropriate assessment-based treatment plans; and the provision of crisis oriented psychotherapy and brief, short-term and long-term psychotherapy and psychotherapeutic treatment of individuals, couples, families and groups, habilitation, psychoanalysis and behavior therapy; all undertaken for the purpose of preventing, assessing, treating, ameliorating and resolving psychosocial dysfunction with the goal of maintaining and enhancing the mental, emotional, behavioral, and social functioning and well-being of individuals, couples, families, small groups, organizations, communities and society.” 

If not licensed in another state the following requirements apply:
  • have an education that includes a masters degree in social work (M.S.W.) with at least 12 semester hours of clinical coursework acceptable to the Department;
  • have at least 3 years of post M.S.W. supervised experience in diagnosis, psychotherapy and assessment-based treatment planning acceptable to the Department;
  • meet clinical examination requirements; and
Note: In most cases applicants must be licensed and currently registered to practice as a Licensed Master Social Worker (LMSW) in New York to complete the supervised experience for licensure as an LCSW. Effective September 1, 2004, the practice of licensed clinical social work is restricted to those licensed or authorized under New York law.

New York LMHC (NY issues LMHC licenses rather than a standard LPC) by endorsement
If an applicant is Licensed in another State, we will apply by Endorsement. Requirements:
An applicant seeking endorsement of a license in Mental Health Counseling issued by another jurisdiction must present evidence of having completed 5 years of licensed practice in the 10 years prior to applying for licensure in New York State. You must have been licensed in the other jurisdiction by meeting the following requirements:
  • holding a graduate degree in counseling or a related field that at the time of completion qualified you for licensure as a Mental Health Counselor in another jurisdiction;
  • completing supervised experience in Mental Health Counseling and psychotherapy that qualified you for initial licensure in the other jurisdiction; and
  • passing an examination acceptable to the New York State Education Department for the practice of Mental Health Counseling.
The applicant MUST be able to provide and help us with the completion of the following:
  • A list of licensed colleagues who are attesting to 5 years of post-licensure experience within the last 10 years prior to application for licensure by endorsement

The supervisors must be able to attest to the following about the applicant when asked to complete the work experience form:

“Mental Health Counseling is the evaluation, assessment, amelioration, treatment, modification, or adjustment to a disability, problem, or disorder of behavior, character, development, emotion, personality or relationships by the use of verbal or behavioral methods with individuals, couples, families or groups in private practice, group, or organized settings; and the use of assessment instruments and mental health counseling and psychotherapy to identify, evaluate and treat dysfunctions and disorders for purposes of providing appropriate mental health counseling services.” 

If not licensed in another state the following requirements apply:
  • Meet education requirements
    • To meet the professional education requirement for licensure as a Mental Health Counselor, you must present evidence of receiving a master's or doctoral degree in counseling from a program that is:
      • registered by the Department as licensure qualifying;
      • accredited as a clinical mental health counseling program of 60 semester hours by the Commission on the Accreditation of Counseling Related Education Programs (CACREP) or another acceptable accrediting agency; or
      • determined by the Department to be the substantial equivalent of such a registered or accredited program.
  • Meet experience requirements; 
    • To meet the experience requirement for licensure as a Mental Health Counselor, you must submit documentation of completion of a supervised experience of at least 3,000 clock hours providing Mental Health Counseling in a setting acceptable to the Department. The supervised experience must be obtained after completion of the master’s degree program required for licensure.
    • Note: When an applicant completes less than 3,000 hours in a setting, at least one-half of the hours must be direct; the remainder may be indirect. It is not acceptable to complete direct and indirect activities in different settings under different supervisors and attempt to combine these hours to meet the experience requirement.
  • Meet examination requirements


  • Interpret full picture of T1-DDAS subscale report and output and create treatment plan alongside the patient for strategies to address these.  
  • Document and maintain concise but comprehensive patient plans within medical record.
  • Work collaboratively with the full BCH Care Team, including a range of clinical and non-clinical disciplines, to navigate through the program and coordinate necessary services that may be identified throughout their journey. 
  • Act as subject-expert within team on MH and diabetes distress and bring forward education topics (“Edu-corners”) to BCH Care Team.
  • Support and administer patient assessments as needed for patient safety, including but not limited to the PHQ2 and 9.
  • Work collaboratively with patients using an empowerment-based approach. 
  • Demonstrate multicultural skills, knowledge and experience, including the influence of cultural issues and providing services with appropriate cultural context. 
  • Demonstrate commitment to the welfare of patients, and a deep understanding of the social determinants of health and the needs of under-resourced populations.
Compensation:
Range: $95,000-$110,000/year. This range reflects the good-faith estimate of the compensation we reasonably expect to pay for this position. Individual compensation decisions are based on job-related factors, including skills, experience, and internal equity.
