As a Lead Director of Population Health defines the clinical program requirements, workflow intent, and success criteria that drive technology enablement for complex care coordination programs directly impacting high-risk patient outcomes.

This role is the connective tissue between what clinical leadership needs and what our systems can deliver. You will work with each program owner — care management, acute escalations, supportive care, and others — to shape the right requirements, bring an informed perspective on the right Epic and non-Epic solutions, document workflows, and define how success is measured. You will partner closely with Informatics and other key stakeholders, moving fluidly between operations, technology, and data. This role owns the what and why; our technical partners own the how.

Clinical Program Design & Enablement:

  • Own definition of clinical requirements for key OSH initiatives keeping patients healthy and out of the hospital, and partner with Informatics and Product from design through go-live and ongoing optimization. 

  • Develop prioritized clinical requirements and use cases to inform Epic and non-Epic technology roadmaps, in partnership with Informatics and Product. Translate clinical program logic and workflows — clinical assessments, MLR thresholds, SDOH screeners, etc. — into clear, actionable requirements for technical partners.

  • Drive the clinical-operations side of complex, multi-team launches. Validate that delivered features meet clinical and workflow requirements (clinical acceptance/UAT), surface issues with technical partners, and ensure nothing falls through the cracks.

Measuring Success:

  • Develop frameworks to measure program success in consistent, scalable ways across a wide range of stakeholders and across programs serving different patient needs

  • Translate plain-language metrics into clear metric specifications for technology and data science counterparts

  • Monitor program performance data, flag anomalies proactively, and partner with Analytics to investigate and resolve issues quickly

Leadership & Cross-Functional Partnership

  • Represent Population Health in cross-functional forums; serve as a trusted thought partner to field leadership, Informatics, Product, Engineering, and operational teams

  • Lead, develop, and retain a high-performing team — giving direct reports real accountability, recognizing wins specifically and publicly, and investing in their growth

  • Develop and maintain executive-ready briefings, strategy docs, and slide content, often from scratch and under time pressure

  • Proactively identify gaps between strategic priorities and current-state capabilities; surface risks early with recommended paths forward

Required Qualifications

  • 10-15 years in healthcare program management, population health, or care delivery operations, with demonstrated ownership of complex, multi-stakeholder programs

  • Deep working fluency with EHR systems (Epic strongly preferred) — enough to translate clinical needs into feasible, buildable requirements, assess technical tradeoffs, and partner credibly with build teams. Direct build/configuration experience is a plus, not a requirement

  • Demonstrated ability to earn trust and influence technical and build teams as a non-owning partner — moving priorities through credibility and shared wins rather than authority

  • Clinical + operational bilingualism: comfortable with clinical program design logic and technical systems (Epic, patient segmentation data, data warehousing concepts)

  • Exceptional written communication: able to produce executive-level materials that are clear, structured, and audience-appropriate

  • Proven ability to drive the clinical-operations workstream of high-stakes go-lives across matrixed teams without losing detail or momentum

  • Strong analytical instincts: can read data, identify what's off, and know who to loop in and how fast

  • People leadership experience with a track record of developing early-career talent

  • High bias toward action; thrives in ambiguity

  • Ability to flag risk clearly and early, including to senior leadership, without over-alarming or under-communicating

Education

  • Bachelor's degree required

  • Graduate degree preferred but not required (MBA, MPH)

Pay Range

The typical pay range for this role is:

$100,000.00 - $231,540.00


This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls.  The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors.  This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.  This position also includes an award target in the company’s equity award program.
