Solara Hospice LLC is seeking a compassionate and dedicated Hospice On Call RN to join our team in Waltham, MA. Responsibilities:
Respond to after-hours patient and family calls, providing clinical guidance and triage support
Conduct urgent visits to assess and manage acute changes in patient condition
Administer and adjust medications for pain management and symptom control
Coordinate with the interdisciplinary hospice team to ensure continuity of care
Document patient assessments, interventions, and outcomes accurately in the EMR system
Provide compassionate end-of-life support and education to patients and their families
Communicate critical updates to attending physicians and on-call medical staff as needed
Requirements:
Active RN license in the state of Massachusetts
Minimum 1-2 years of hospice, palliative care, or home health nursing experience preferred
Strong clinical assessment and critical thinking skills
Experience with pain and symptom management in end-of-life care
Proficiency with electronic medical records (EMR) documentation
Valid driver's license and reliable transportation for home visits
Excellent communication skills and ability to remain calm under pressure
