Responsibilities:
Build and maintain relationships with hospitals, skilled nursing facilities, physicians, and other referral sources to drive hospice admissions
Educate healthcare professionals, patients, and families on hospice eligibility, services, and the benefits of end-of-life care
Assist with the coordination of timely hospice evaluations and admissions
Collaborate with the interdisciplinary care team to ensure smooth patient transitions and continuity of care
Serve as a community ambassador, representing Care Plus Hospice Care at outreach events and healthcare networking opportunities
Maintain accurate documentation of referral activity, patient interactions, and care coordination efforts
Provide ongoing support and communication to families navigating the hospice process
Requirements:
Prior experience in hospice, palliative care, home health, or a related healthcare setting strongly preferred
Proven background in healthcare sales, community outreach, or clinical liaison roles
Strong knowledge of hospice regulations, Medicare/Medicaid eligibility criteria, and end-of-life care principles
Excellent interpersonal and communication skills with a compassionate, patient-centered approach
Self-motivated, organized, and able to manage a territory independently
Valid driver's license and reliable transportation required for local travel throughout the Scottsdale area
